We are in the process of preparing our annual Book of Remembrance for 5786.  The book provides an opportunity to honor our beloved departed.  Names will be listed alphabetically by family.  Once again, instead of a per name price, we are requesting a minimum contribution for each member family listed as follows-:

1-5 Family Members ....$36.00
6-10 Familiy Members....$54.00
11 and UP Family Members...$72.00

Of course, additional contributions are always welcome!

Please review the listings from last year and then complete and return this form.  If you need assistance reviewing last year's listing for your family, please call the GHC office at 914-693-4260, and we will be happy to help.  The DEADLINE to return this form and your payments to the GHC office is SEPTEMBER 5, 2025
